You must bring a lab requisition to your appointment. Our lab is not able to provide requisitions. If you need a requisition, please book an appointment with one of our physicians instead.
After you complete your appointment request, you will receive an immediate email confirmation. The email contains a link if you subsequently need to cancel. You will also receive a reminder email one day prior to the booking.
